Total sales amount includes 6% KY sales tax. Gift cards can only be used for new individual memberships and are applied to consecutive monthly membership only. Gift cards cannot be applied to existing memberships. Gift cards cannot be used for add on classes, seminars, etc.  Membership begins at the date the first class is taken, and expires 1, 3, 6 or 12 months after. Example: the recipient receives a 3 month membership card on December 25, 2023 and takes their first class on December 28th. Their membership will begin December 28th, 2023 and end on March 28th, 2024. Membership cannot be paused or put on hold. Gift cards are non-refundable and once activated memberships are non-transferrable. Minors wishing to use a gift card must have a waiver and membership form signed by a custodial parent or guardian on file before beginning classes.
